The skin and other tissues of the ageing face follow gravity and start sagging. There is loss of volume and the muscle activity causes wrinkling.
Whereas the descent of the facial tissues will need repositioning, the volume loss can be corrected with fillers. The choice is difficult nowadays with a wide variety on offer. But if one follows a logical approach to the problem it becomes clearer: The process is dynamic, so the filler should be easy to adjust and replenish. The best substance is therefore Hyaluronic Acid, which has a low risk profile, stays a limited time and can be renewed any time. The biological alternative is own body fat, which is essentially the largest part of the tissue lost. The downside is that it requires a small surgical intervention to harvest.
The cause of wrinkling can be muscular activity. Botulinum Toxin is the obvious choice, an agent which has proven to be safe in treating spastic conditions in small children or paralysed patients in very high doses.

Volume deficits we will address by precisely adapted Hyaluronic Acid injections. The effect lasts between 6 and 9 months. Exceptions are the lip region, which shows a shorter time and the décolleté region where the effect can stay longer.
Wrinkles due to muscle activity will be treated with Botulinum Toxin (eg Botox). It takes about 3 - 4 days to have an effect, which will normally last around 4 - 6 months.
